What difference does it make if you buy a higher priced policy for your medical insurance? The government makes the decision on your treatment at the hospital. Not the surgeon or MDs.
The heart surgeon did not make the decision to send me home half way through my heart operation, the government did. It didn't matter what I was on. Medicare, Medicaid, private, or expensive exchange policy. Or subsidized. It's all the same if the government wants to cut you they will. There is no standard of guaranteed care even if you have money to buy private insurance.

They reschedule you and two years later you still haven't got the rest of the operation. Get it? That's the same as a two year wait.
Yes they can make you wait two years even if you have a better policy.
